Ok, im now convinced its my Vera being a PITA.
All these conditions are met and none of the 3 Z-Wave fans turned on: 
this is yet another Reator sensor that has been working 100% for months.
Cycling the reactor sensor produced the desired result.
Screen shots are probably the least useful thing you can show me for stuff like this. I feel like a broken record. Logic Summary. Always Logic Summary.

I didn’t bother because I know the logic is good (I do have it captured and can post later if you are interested) - I think my VP was in a downward spiral as the failures seemed to be happening more frequently than normal.
I even tried to install the Netatmo plugin (from scratch) and the install process did not go normally and I had to manually remove it as it failed. I then rebooted my Vera and this time it installed successfully. My Vera had only been up for 8 days, since the last reboot, so I was not really expecting it to be an issue.

Btw, I haven’t had any state change failures since rebooting my Plus. As a result I’ve reinstated weekly scheduled reboots (I had hoped Monthly would be enough).
This summary shows the “Fans On is TRUE” activity and “Off Logic is TRUE” activity running at the same time… look at the logged “runscene” entries, and the effect on the devices (“devicewatch”).
You may want to make sure your logic is interlocked sufficiently that both can’t fire together.
Also, set MaxEvents higher on this ReactorSensor… 100 or 150 maybe, so you can capture more history. May be useful.
That odd as I wasnt seeing any logic conflicts in the main status page.
The main status page only updates about every 2 seconds. That’s a Vera limitation–that’s as fast as UI7 gets updates from the controller. So anything that happens is less than two seconds may not be evident on the status page. The only way you would know is (a) the effect on devices, and (b) the state times change in the status view, but that’s a really subtle change (e.g. the state time may change from 18:00:14 to 18:00:15).

So we can see the setpoint being changed at 05:00:00.217 as I’d expect, but then at 05:03:59.101 it goes back to being 18.0
Any thoughts as to why this would be? Are we losing commands (seems unlikely) but what else do we need to see (I can create a logic report but it will have changed by now)

OK. Well a device that gets sent a command, and then reverts back to the prior, suggests to me that the command did not make it to the device, and a later update/poll of the device retrieved the unchanged prior value.
So what’s the diagnosis? UI7 or just ‘stuff’?

Hard to say. The way you reformatted the log file, I can’t actually see what went out the thermostat, but if works some of the time, I have to assume Reactor is sending the parameters you’ve instructed.
I’d go the route of the pulse repeat, to make sure the command gets resent if the setpoint doesn’t change. As I said, you need to be really careful that your logic is properly interlocked (two things can’t run at once), and that the conditions test whatever target state the activity will try to set (that is, if you are trying to turn a light on, the activity will have an action to turn the light on, and the condition that controls that activity should test to see if the light is off, so the activity is only run when necessary). You’ll need to upgrade to 3.5 to get the pulse train capability.
Nothing else will be impacting the heating at that point.
When you say ‘pulse repeat’
Just set the pulse to (say) 5 minutes? I just added a delay 5 minutes and resend in the activities, but I suspect the other way is rather more elegant?

Delay and resend may also work, if relying on a single retry is good enough. Certainly has the advantage of being simple.
If you go pulse, set the pulse length to something short, just a couple of seconds, then choose “repeat” and enter 300 for the “repeat after” time. You may also want to add a max count, just as a safety mechanism (so it doesn’t retry indefinitely, it retries, for example, 10 times before giving up).
